INSTRUCTIONS FOR THE USE OF ICE PACKS
These packs remain the property of Axiom Veterinary Laboratories
and are lent to practices for the transmission of samples only to Axiom.
Loss of any part of this kit or failure to return to Axiom within
14 days
(except by prior agreement) will incur a £50.00 +VAT charge
to your account.
Contents:
1 x COOL BOX
1 x COOL TRANSPORT CONTAINER
1 x POLYSTYRENE BOX
2 x ICE BLOCKS
Protocol:
Phone the lab 01626 355655 request a freezer pack which will be
sent by courier at the special cost of £8.00
Take blood sample, separate plasma or serum as per protocol and
freeze.
Freeze the cool transport container and 2 ice packs for a minimum
of 24 hours.
Contact the lab 01626 355655 to arrange a courier pick up for a
special frozen sample cost £8.00
Prior to despatch, place frozen plasma/serum in cool transport
container and place in polystyrene box. Place box inside cool box placing
ice packs on top. Keep in freezer until despatch.
Do not send over a weekend or on a Friday.
Courier charges of £16.00 + VAT will be added to your account. |
